Discharge Information
- Your doctor will decide as to when you have to be discharged
- The doctor will write the discharge date in the case sheet and communicate the same to you and the nursing station.
- The billing department will prepare the final bill for settlement
- On settlement of the dues to the hospital, you may plan to get discharged.

In case you are an Insurance patient, our Insurance Department will give the clearance. If any dues are there to the hospital, viz. diet expenses, telephone charges, etc, the same will be notified to you and you may pay such amount to the cashier.

- Please complete the discharge feedback from that would be given to you at the time of discharge.
- Your feedback will help us to serve the patients better.
The following is the procedure for discharge at SHRC :

Your Consulting/Attending Physician will make an entry of discharge in your case folder.

The Resident Doctor will then fill in your discharge card with details and the further course of treatment. If you need to come back for a follow-up assessment, it will be indicated. Drug prescriptions and other instructions will be given.

A Discharge Summary is created from the Discharge Card (Summary)

Your folder containing the relevant papers and vouchers will be sent to the Billing Department for appropriate billing.

The final bill is readied after the deposit is taken into account. On receipt of the final bill, payment has to be made at the billing counter on the Ground Floor.

Make sure to make your appointment on the "follow-up" date given at the OPD counter in the Main Lobby, so that you are not delayed for your appointment and your case history is kept ready and your doctor informed.
